Identifying and Assisting Consumers at Risk

This tool can be completed by support persons to help identify risk factors for involuntary psychiatric hospital admissions for the individuals they support. The tool can also help identify community resources and activities that may be useful to the individual. Once hospitalization risk factors and community resources have been identified, the support person can work with the individual to develop a plan for reducing risks and strengthening community supports.
